My girl was born 0608, form submitted on 13 Sep morning, still not yet received the form...so worry.

I just called and they confirmed receive my application form, but can't tell me when is the interview date.

I receive no form yet and I also handed it in person on 13 th morning...don't worry...maybe just too many admin works to handle and late to send out only....
