May be I can share what I know as I have helped at lunch time in school for a week, the flow for P.1 and P.2 are like that:
when the lunch bell rings, the worker will put food on the tray with the lid on, then place them on the table in order(table 1, 2...etc), when the kids come up and sit in sequence, the worker will take the lid off.
Sometimes if the kids are late for some reasons, there is a chance that the food may be cool down a bit even with the lid on. But it doesn't happen often.
In my opinion, the school is really doing a great job, the flows are in good order, they have lots of spare food so kids can have extra(second or third rounds), they provide soups at least twice a week, you will be surprised that lots of kids love soup and keep getting refills. Of course, the down side is there are lots of spills and the workers have to keep mopping floor and wiping tables. But they don't mind doing this hard work.
Overall, their food are good, but there is always a trade off between healthy food and yummy food, I think the school and the food provider are trying their best to get a balance on that.
I also think it's an excellent idea to have parent helpers there, not only we can understand more about the process, we can also observe our kid's behaviour at school. I lost my voice during that week because I kept telling the kids....please eat a little bit more, please try, you will be hungry very quickly, won't you like more vegetable too? want more food? want more soup?...........oh you don't like beef? I have extra pork here......ha ha, it's like I have 1,000 more kids.
