I am not living in net 41 so I could not give you any advice in filling the form.

May experienced parents here who has experience in net 41 and shed 松松
some light to fill the form for her poor nephrew

Don't bother to think of any private school for your nephrew first.  I won't doubt your financial ability however there are lots of good primary schools in net 41.

Save the $ to nuture this brilliant kid.

LS is top school however don't put too many eggs in one basket if filling LSLSLSLS on first 1..2..3..choices.
