i got 2 extra application forms bought from HKPS (Kln Tong) as my friends got them already. I've already paid $30 for each form (so you don't need to attach $30 cheque to the school again), I just want to give it out at the same price.
If you are ok to collect it at 8:30 a.m. in Mei Foo MTR on 5th Nov (Wed), pls pm me your contact #.

Is your form downloaded from internet?  I downloaded from internet but there is no number on the form.  I called the school today and they responded that there is no number printed on the form downloaded from internet.
