WANTED: ＣＣＫＧ申請表 Please Help!!!!

I miss the date to collect the application form, does anyone have an extra form?  Since we need to send in the form in person tomorrow morning.  If anyone has an extra form, we can meet outside the school tomorrow morning.  Please help and PM me if you have an extra form. Thanks very very  much.

there's slim chance that the school may accept late application,
you can try to give them a call, anyway

Yes, deadline was 6 Nov 9am to 10am.  The details of application stated clearly no forms will be welcomed afterwards ga.
